The Case for Continuity in Functional Medicine Care

I work with women who are smart, health-literate, and exhausted from trying to manage complex symptoms on their own.

They’ve done labs.
They’ve tried diets.
They’ve seen multiple specialists.

And they’re often told some version of: “Everything looks normal.”

These are the women that functional medicine is supposed to help. And it’s also why the structure of care matters just as much as the tools we use.

Functional medicine is not designed for crisis visits or flare-only care. It doesn’t work well when you drop in, make a few changes, and disappear for six months. Bodies don’t heal that way, especially bodies dealing with immune dysregulation, hormonal instability, gut inflammation, or metabolic stress.

The Problem With One-Off Visits (Even Good Ones)

A single functional medicine visit can be thorough. It can be insightful. It can even be validating.

But for most complex conditions, it’s not sufficient.

Here’s why: physiology is dynamic. When we intervene—whether through nutrition changes, microbiome support, hormone modulation, or nervous system regulation—the body responds. Sometimes quickly. Sometimes slowly. Sometimes in ways we don’t fully predict.

Without follow-up, there’s no way to:

  • Assess whether an intervention is working for your physiology
  • Adjust dosing, timing, or sequencing
  • Interpret symptom changes in context
  • Prevent over-treatment or under-treatment

This is especially true when we’re working with systems like:

  • The gut microbiome
  • Immune signaling
  • HPA axis and cortisol rhythms
  • Estrogen metabolism
  • Inflammatory pathways

These systems don’t respond to one-time inputs. They require monitoring and adjustment.

Functional Medicine Is an Iterative Clinical Process

In my practice, functional medicine is iterative.

We investigate.
We intervene.
We assess response.
We refine.

That feedback loop is where progress happens.

Without continuity, patients are left to troubleshoot alone, often second-guessing whether symptoms mean something is working, something needs to change, or something new is emerging.

With continuity, we can respond in real time and prevent unnecessary backtracking.

How I Think About Healing: Investigation → Resolution → Maintenance

All of my care paths are structured around how complex conditions actually improve.

Investigation

This phase focuses on identifying drivers, not just diagnoses.

That often includes advanced testing such as:

  • Comprehensive stool testing to assess microbiome composition, inflammation, and immune signaling
  • Hormone testing that reflects real physiology, not isolated snapshots
  • Targeted metabolic or inflammatory markers

Investigation gives us direction, but it doesn’t resolve symptoms on its own.

Resolution

Resolution is where most of the work happens, and where one-off care fails.

This phase requires:

  • Ongoing adjustment based on response
  • Education so patients understand what we’re changing and why
  • Pacing that supports the nervous system

Resolution is rarely linear. That’s normal. What matters is having a structure that can adapt.

Maintenance

Once stability is achieved, the goal shifts.

We’re no longer chasing symptoms. We’re protecting gains, preventing relapse, and supporting long-term resilience. Maintenance is active care, just at a different intensity.

Single Visits: When They Make Sense

Single visits are appropriate for women who:

  • Want a second opinion on labs or a treatment plan
  • Have a specific, contained question
  • Are already stable and not seeking active treatment
  • Need clarification, not ongoing support

They are not designed for unraveling multi-system patterns or managing change over time.

The Monthly Care Plan: Why It’s the Recommended Path

For most women who come to functional medicine, ongoing care makes the work effective.

The Monthly Care Plan provides a structured, responsive framework that allows us to do exactly that.

This care path includes:

  • One 60-minute functional medicine visit each month, conducted in person or via Zoom
  • A revised health action plan after each visit, reflecting current findings and response
  • New or follow-up lab orders as clinically indicated
  • Unlimited secure portal messaging for questions, troubleshooting, and guidance between visits
  • Optional add-on visits with a health and nutrition coach for additional support
  • A dedicated “what’s next” conversation when you’re ready to transition out of monthly care
  • The option to continue with a Patient Portal–based membership for maintenance and ongoing education

It’s the model I recommend most often because it aligns with how complex systems actually stabilize and improve.

Choosing the Right Path

If you’re considering functional medicine, consider how much support you’re looking for.

Are you looking for clarity or active resolution? A short consultation or a structured process?

Take time to read through the care options, reflect, and choose the model that aligns with how your body, and your life, actually work.
