Your Gut Microbiome and Fertility: What Preconception Care Is Missing

When a woman comes to me thinking about pregnancy, I’m looking at three things that standard preconception care rarely addresses together: her metabolism, her nutrient status, and her microbiome. All three have a direct bearing on whether conception happens, whether the pregnancy stays healthy, and what biological foundation she passes to her child.

Most preconception conversations focus on folate and stopping birth control. That’s a starting point. Here’s what I’m actually investigating.

Metabolic Health and Egg Quality

Insulin sensitivity matters for fertility in ways that don’t show up on a standard panel until things have gone pretty far. Blood sugar dysregulation — even mild, even subclinical — can impair the hormonal signaling that supports regular ovulation, healthy implantation, and a viable pregnancy. It’s also one of the primary and underappreciated drivers of gestational diabetes.

I assess metabolic markers in preconception patients routinely, because this is a window where we can actually move the needle before it becomes a problem.

Nutrient Status: Testing Rather Than Assuming

Standard prenatals cover the basics, but they don’t tell us what’s actually happening in your body. We test specific markers — folate, B12, iron, vitamin D, omega-3s, zinc — because egg quality and uterine lining integrity depend on having the right building blocks available at adequate levels. Where there are gaps, we address them directly. Supplementing a prenatal for a woman who may have absorption issues or specific deficiencies is not the same as knowing her status and correcting it.

The Microbiome: A Generational Investment

This is the piece I find most underappreciated in preconception care. Your future child inherits their gut microbiome from you. Colonization begins in utero, accelerates through the birth canal, and continues through early feeding. The foundation of your child’s immune function, metabolic health, and even mental health is being shaped by your microbial ecosystem before they’re born.

What This Looks Like in Practice

Then we reassess, because this work is iterative.
