Three Tests Every Woman with Autoimmunity Should Get Annually

When you live with autoimmunity, your body is constantly working to find equilibrium between defense and overreaction. The key to staying in remission—or preventing flare-ups—lies in understanding the internal environment that shapes your immune system every day.

Three simple tests can offer invaluable insight into how your gut and immune system are communicating:

  • A Microbiome Map
  • An Intestinal Permeability Test
  • A Vitamin D Blood Test

These labs tell a powerful story about inflammation, resilience, and immune balance. Here’s why I recommend each one—every year.

1. A Microbiome Map

Order Test

The community of microbes in your gut—about 4 trillion of them—plays a massive role in regulating inflammation throughout your body.

When we look at a microbiome map, we check on two key players:

  • Anti-inflammatory microbes, like Faecalibacterium prausnitzii. These bacteria produce butyrate, a short-chain fatty acid that helps calm inflammation in your gut, joints, and brain.
  • Inflammatory microbes, often those that produce lipopolysaccharide (LPS) endotoxins. These compounds can trigger a full-body inflammatory response and even spark autoimmune activity in susceptible people.

But here’s the critical piece: the magic isn’t in the test; it’s in the interpretation.

AI-generated lab recommendations and automated reports are usually generic, overwhelming, and often flat-out wrong for your specific patterns. This is why working with a Certified Microbiome Analyst matters. A trained human expert can read the nuances—microbial ratios, metabolite patterns, ecological shifts—and translate them into personalized, strategic action steps.

A healthy, butyrate-producing microbiome is one of the most powerful tools for modulating your immune system, and a skilled reviewer will show you exactly how to build it.

2. Intestinal Permeability

Order Test

Your small intestine is lined with a single layer of cells—one cell thick—that decides what gets into your bloodstream. That lining is supposed to be selectively permeable: allowing in nutrients and beneficial molecules like butyrate while keeping pathogens and toxins out.

But when it becomes too permeable (a state called hyperpermeability or “leaky gut”), those LPS endotoxins and other inflammatory triggers can slip through. The result? Widespread inflammation that can manifest as anxiety, bladder irritation, joint pain, or brain fog.

Since leaky gut is highly correlated with autoimmune conditions, maintaining a healthy intestinal barrier—supported by balanced microbes and mucosal protection—is foundational to immune health

Here’s where many people go wrong:
You cannot assume you have leaky gut. You must test.

A clinically significant permeability issue requires a minimum of six months of targeted treatment, followed by maintenance support. Treating when you don’t truly have permeability problems not only wastes time—it can make things worse.

This test gives us clarity so we can address the actual root cause, not a hypothetical one.

3. Vitamin D

Most of us are low in vitamin D. Between limited sun exposure and sunscreen use (both for good reason), deficiency is incredibly common.

But vitamin D is much more than a bone-support nutrient—it’s an immune regulator. Adequate levels help your innate immune system calm overactive cytokines and increase regulatory T cells and Innate Immune cells, the warriors and peacekeepers that keep autoimmunity in check.

If you want to learn more about the role of T cells in autoimmunity, read my blog on it.

If your vitamin D is low, your immune system is more likely to overreact. Checking and optimizing your levels at least once a year is essential for long-term autoimmune balance.

The Bottom Line

Your immune system reflects the health of your inner ecosystem. These three tests—microbiome, permeability, and vitamin D—give us a window into that ecosystem and help us target the real causes of inflammation, not just suppress the symptoms.
