Perimenopause 101: What’s Happening and What Helps

What’s happening to my body?

The sleep. The mood swings. The weight changes. Brain fog. More frequent periods. Hot flashes. “Is this hormones? Is this…perimenopause? Why does no one teach us this? Why wasn’t my doctor helpful? Where’s the guidebook?”

You’re not alone—and you’re not imagining it. We aren’t taught about perimenopause (though we should be). And unless your doctor chose additional training after medical school, they probably weren’t taught much about it either. So we turn to podcasts, books, and social media—helpful, but often overwhelming and conflicting.

Here’s the good news: there is a way to make sense of it.

In my practice, we have what I call “the perimenopause conversation.” It’s my way of giving you a personalized map through this transition, shaped by your story, genetics, symptoms, and goals. Over the next couple of months, I’ll share pieces of those guidebooks here—so you have practical clarity, step by step.

Today we’ll cover:

  • What perimenopause actually is
  • Common symptoms (and what’s really driving them)
  • Why the same symptom can have different root causes

What exactly is perimenopause?

Perimenopause isn’t a disease. It’s a natural transition every person with ovaries goes through.

  • Perimenopause is the lead-up to menopause: the ovaries still cycle hormones, but levels begin to shift.
  • Menopause is the point when periods stop. After 12 months without a period, you’re considered post-menopausal (most of us still say “menopause”).
  • Timing: Often starts in your 40s (sometimes late 30s) and typically lasts 4–8 years (shorter or longer is still normal).

The usual hormone pattern:

  • Progesterone gradually trends lower over years.
  • Estrogen can be wildly up-and-down (even high) in mid-perimenopause.
  • In later perimenopause, both estrogen and progesterone are often low.

Every woman’s hormone trajectory is unique. Patterns are guides—not verdicts.

“Is this symptom perimenopause…or something else?”

Many symptoms can be driven by perimenopausal hormone shifts—but they can also have other root causes. That’s why a clear map matters.

Bottom line:

If you’re in your 40s (or late 30s) and checking a lot of these boxes, perimenopause may be part of the story—but rarely the whole story. Most symptoms have more than one root cause. A thorough, personalized assessment is how we build a plan that actually works.

What this looks like in my care

In our Perimenopause Conversation, we:

  • Map your symptom pattern to likely hormone trends and non-hormonal contributors
  • Consider sleep, stress, thyroid, insulin resistance, inflammation, gut health, and nutrient status
  • Clarify your goals (e.g., sleep through the night, steady mood, lighter periods, clear thinking, comfortable sex)
  • Craft a personalized guidebook you can actually follow—adjusted as your body changes

You’re not broken. You’re in transition. The right map turns confusion into confidence.a thrive, inflammation quiets down, and the “pesky bugs” fade out.

What’s next in this series

In upcoming blogs and emails, I’ll share:

  • How to track cycles and symptoms to spot your stage
  • Sleep + mood support in early vs. late perimenopause
  • Period problems: what’s hormone vs. what needs further evaluation
  • Nutrition strategies for estrogen swings, insulin resistance, & inflammation
  • Evidence-informed options (lifestyle, supplements, HRT) and how to personalize
